Patients are responsible for co-payments at the time of service. They will not be billed. |

Patients are responsible for Referrals at the time of service or full payment is expected. |

Patients are responsible for providing to the health
care provider, to the best of his or her knowledge, accurate and
complete information about present complaints, past illnesses, hospitalizations,
medications, and other matters relating to his or her health. |

Patients are responsible for reporting whether he or she clearly
comprehends a contemplated course of action and what is expected
of them. |

Patients are responsible for following the treatment plan recommended
by the health care provider. |

Patients are responsible for his or her actions if he or she refuses
treatment or does not follow the health care provider’s instructions. |

It is understood that patients assume the financial responsibility
of paying for all services rendered whether through third party payors
(his or her insurance company) or being personally responsible for
payment for any services which are not covered by his or her insurance
policies. |

Patients are responsible for providing complete, accurate and
current billing and insurance information. |
 |
Patients are expected to cooperate with all Center personnel and
ask questions if directions and/or procedures are not clearly understood. |
 |
Patients are responsible for following health care and facility
rules and regulations affecting patient care and conduct. |
 |
Patients are responsible for arranging transportation to and from
the facility. |
 |
It is the patient’s responsibility to keep appointments, or
cancel them 24-72 hours in advance, whenever possible. |
