What to bring with you to your first appointment.
If your appointment is for an initial office visit, please bring your insurance cards along with your driver’s license so we may verify coverage. If paperwork was mailed to you from our office, please bring this on the day of your appointment. Do not mail this information back to the office. (We also request that you bring all recent laboratory work, scans, and x-rays to your initial appointment).Please download and fill out, prior to your first visit, all 5 pages of our new patient forms.

HMO Patients

If you are a HMO patients, you must bring your referral form from your primary care physician. Also, please be prepared to pay a co-payment at time of visit.

Appointment Cancellations
If you need to cancel an appointment, we require that you notify us 24 hour in advance of our regular office hours. We reserve the right to charge for appointments canceled or broken without 24 hours advance notice. Occasionally, hospital emergencies will result in a change in your appointment. We will notify you as soon as possible when this situation arises.
